- This is a standard application form for all courses offered by the institute. please read carefully before completing it.
- This form should be completed by all applicants whether new or former students.
- All sections of the form are to be filled in full.
- Certified photocopies of all academic transcripts and certificates of educational qualifications must be attached. public servants shall have endorsement from their superiors or the human resource manager.
- Applications not completed properly or incomplete will be rejected.
